We’re looking for an enthusiastic, engaging, knowledgeable and dedicated Transitions Youth
Worker to join our team based with Bristol Mental Health (Adult Services) and help us deliver a robust package of care that will support the recovery of those aged 16-25 who are working with statutory mental health services.

The post will deliver structured support to young people in order that they engage with internal and external services along with community provision. To provide activities in order to promote good mental health and wellbeing, as well as looking at strategies to enable the young person to feel empowered to make positive choices.

Experience of working with complex individuals, a good working knowledge of services within the Bristol area, and an ability to engage with hard to reach individuals are all essential requirements of this exciting post.

Building on the interests and resources that young people have is fundamental to the role, along with a commitment to improving outcomes for young people. Working as part of OTR you will develop a strong relationship with services to enable referrals and engagement with the wider Team.
